Changes in DEFRA regulations mean that owners will not have to sign the declaration at Section IX in the passport until the horse is exported, administered with a medicine which should never be administered to a food producing animal, or consigned for human consumption. However, should you wish your equine NOT to be for human consumption we suggest you sign the appropriate section upon receipt of your passport and have your vet countersign it as a competent authority.

NOTE: All persons signing this Application agree to be bound and abide to the Rules and Regulations of the United Saddlebred Assoc. UK Ltd from time to time amended. Responsibility for correctness of information submitted herein rests entirely with the applicant. USA-UK is not responsible for any erroneous statements made upon the Application blank. Should any animal be registered or transferred through error, misrepresentation or fraud, the entry and transfers thereof shall be void, together with any entries that have been made of progeny of such animals. Proven parentage (non-broken) by DNA required for Main Registry. Dual registration with the American Saddlebred Horse Association (ASHA) is recommended.
It is strongly recommended that equines registered with a non-Saddlebred breed society obtain their passport from their relevant breed society, as the United Saddlebred Association can only verify SADDLEBRED parentage. In the event of the passport being issued by USA-UK, the breeding details have to be validated by the breed society AFTER the passport has been issued. Please use CAPITAL letters as we need to be able to read your details. If any details are unknown, please put “NOT KNOWN”. Estimate year born, if foaling date not known. Height & colour, plus owners details must be included. The identity section overleaf must be completed by a Veterinary Surgeon or competent authority. If you have a current vaccination certificate with this ID completed by a Vet, please submit this (it will be incorporated within the passport). Photo’s are optional and can be included in the passport.

INSTRUCTIONS
Please complete the application form, in accordance with the instructions given. If you experience any difficulties in understanding what is required, please contact the Registrar. If you have a current vaccination certificate (completed by a vet.) please submit this as well, as it will be incorporated within the passport. Once the form is completed, please send it, together with a cheque for the correct amount, made payable to USA-UK. The form must be returned to the Registrar – USA-UK, 36 Philpot Square, Peterborough Road, London, SW6 3HT. Please ensure that you print clearly in block capitals – you may know your address or stud prefix, but our staff need to be able to decipher these words accurately – so please BE CLEAR!

Photo’s
2 colour photo’s, one of each side of the equine, MAY be sent (no riders, saddles or harness please!) This is mandatory for or skewbalds/piebalds (pinto’s and paints) who should also include a photo of the front and rear view. If submitted these will be included within the passport and validated photo’s can also be added after the passport has been issued.

Section A – DETAILS OF EQUINE
Name - Prefixes / suffixes must only be used if they are owned by you / the breeder and registered with the Central Prefix Registry.   If an equine is bred by a breeder with a registered prefix, that prefix should be given to the equine on registration. Duplicate (names: Saddlebreds only) or distasteful names are not permitted. A distasteful name is one, which, in the opinion of the USA-UK Committee, is distasteful and / or likely to cause offence, in which case, the name will not be permitted.

Breed – Please put specific breed if known (e.g. Saddlebred, Partbred Arab, Welsh etc.). Do NOT put ‘heavy type’ or ‘Cob” etc. If breeding is not known, please put NOT KNOWN. Verification of breeds can only be made by the respective breed society. For example if you are registering an Arab with USA-UK we will only state “Breeding – Arab, non-verified”. DNA details (if applicable) are now included in the passport – please give reference number & name of lab.
AMERICAN SADDLEBREDS ONLY – Please show USA-UK Registration Number for both sire and dam. If one, or both, are not USA-UK registered, please submit a copy of registration (other ASB society) showing up to 4 generations of pedigree (if available). American Saddlebred sire, dam and foal being registered must be DNA tested for proof of parentage before acceptance onto the USA-UK Main Registry (Purebreds, with proof of parentage. Kits available from the Horse Registrar. If a Saddlebred is registered with: ASHA, ASHA Australia, CRLC Canada or SHBSoSA, South Africa then proof of parentage is accepted, but the DNA Profile Number and the Name, address and fax number of the lab must be submitted in order for entry onto the Main Registry (for proof of parentage for future generations).

Sex – filly or colt (if under 3 years of age), gelding, mare or stallion.
Date of birth – this MUST be completed. If you do not know the exact date, please enter an approximate date. In this case, enter either a year, or a year and a month. AMERICAN SADDLEBREDS ONLY – please submit covering certificates where-ever available. From 2005 these have been replaced with Stallion Service Reports.
Colour – This must be the main body colour. In the case of Skewbalds and Piebalds, please give the base body coat (e.g. Bay and white). Pinto’s / Paints MUST submit photo’s with their application.
Place of birth – Please enter the county (if born in UK) or country and district (if born abroad). If place of birth is not known, please enter ‘not known’.
Height – Please give approximate adult height measured to wither (highest point at base of mane). May be entered in either hands (units of 4 inches) or centimetres, Please ensure you specify which!
Equine’s pedigree - Please enter the name and registration number of AT LEAST the sire and dam. If these are not known, please enter ‘not known’. If the sire and / or dam are known, but not registered, please enter as much information as is known e.g. approx. Date of birth, owner’s details etc (even if only Dobbin, 1989, Mr Kentucky, Maidstone, Kent).

Section B – RECORDING OWNER
This must be completed and the owner must sign the application form. Please include nationality of owner.

Section C – SLAUGHTER FOR HUMAN CONSUMPTION
Changes in DEFRA regulations mean that owners will not have to sign the declaration at Section IX in the passport until the horse is exported, administered with a medicine which should never be administered to a food producing animal, or consigned for human consumption. A Veterinary Surgeon can countersign as the competent authority.

Section D – KEEPER
This part of the application form to be filled in if the keeper is NOT the owner – e.g. if you keep your horse at a livery yard.

Section E – EQUINE IDENTIFICATION
This MUST be completed by a veterinary Surgeon or a Competent authority, unless your equine is micro-chipped – then the owner can complete the ID section, OR you submit a current vaccination certificate which has the ID/silhouette completed by a vet. If you already have a DEFRA passport and are simply registering your equine with us, please submit this as well – in these circumstances the ID section of this application blank does not have to be competed by a Vet.
